The world of Medical Insurance and Vision Care Plans is a complicated and confusing landscape to navigate. Our office staff is trained to help you obtain accurate estimates in a reasonable timeframe to help you in making your medical decisions. We appreciate your patience in regards to looking up this information. Getting estimates of cost can sometimes involve multiple phone calls with wait times that can exceed an hour. We take pride in our customer service, working hard to get accurate estimates in a timely manner.

CANCELLATION AND OTHER POLICIES


Cancellation

Failure to keep your scheduled appointment prevents us from providing the best care to our other patients. Please call at least 24 hours in advance to cancel an appointment. There is a $25 charge for missed appointments and/or failure to cancel an appointment with fewer than 24 hours notice.

We are currently in the process of becoming a preferred provider for many insurance companies. Even if your insurance company is not listed below, please contact our office to find out if we can work with your insurance as an out-of-network provider. Many insurance plans will cover the bulk of an exam even out-of-network.

Referrals

We are in network for the above insurance company’s PPO and POS plans. We are also in network with many of their HMO plans. If you have coverage with an HMO, please call the number on your card and verify with customer service that we are an approved provider. Please request that your selected primary care physician send us the necessary referral. If you have further questions about this process, then feel free to contact our office for further instructions.

Payment

All co-payments are due at the time of the appointment. We accept credit cards (Mastercard, Visa, American Express, and Discover), checks, and cash.

INSURANCE PLANS



Medical Insurance provides for an eye examination or evaluation when the nature of your problem is medical. Medical conditions such as dry eyes, cataracts, or eye infections are billed through your medical insurance.

Vision Care Plans are savings plans designed for routine eye care in absence of any medical problems with the eyes. These plans assist in discounting routine evaluation, glasses, and contact lenses.
